If your son or daughter would like to take part in any of the above after-school activities please tick the relevant box and provide the necessary information below..

A nominal cost of £1.00 per session (£10.00 for the spring term) for all teacher led activities will need to be paid at the time of applying. This money will be used to provide necessary equipment for after-school activities.

Spaces will be limited and will be allocated on a first come first served basis.

I understand that due to unforeseen circumstances it may be necessary to cancel some clubs at short notice.
